Your rights

As a V-Care patient, you have the right to:

  • Respect. Be treated with courtesy and dignity, without discrimination. Our nondiscrimination notice explains this in full.
  • Privacy. Have your health information kept confidential and shared only as the law allows. Our HIPAA notice explains how we use and protect it.
  • Information. Get clear explanations of your health, your options and their likely benefits and risks, in words you understand.
  • Consent. Agree to or refuse a test or treatment, after hearing what your choice could mean for your health.
  • Your records. See and get a copy of your medical records, and ask us to correct something you believe is wrong. Use the records request form.
  • Communication help. Ask for help in another language, or with hearing, vision or other needs, so you can understand your care. See our accessibility statement.
  • Costs. Ask what your care will cost before you receive it. If you're uninsured or paying for yourself, you can ask for a Good Faith Estimate.
  • Speaking up. Raise a concern or complaint without it affecting your care.

Your responsibilities

To help us care for you well, we ask that you:

  • Give us accurate information. Tell us about your health, medicines, allergies and past care as fully as you can, and let us know when something changes.
  • Follow your care plan. Follow the plan you and your clinician agree on, and tell us if you can't, or if you have questions about it.
  • Keep your appointments. Arrive on time, and let us know as early as you can if you need to cancel or reschedule.
  • Treat others with respect. Be courteous to our staff and to other patients.
  • Take care of the financial side. Keep your insurance details up to date and pay your share of the cost, such as copays. If you have a question about a bill, ask us.

Raising a concern

If something about your care doesn't seem right, tell us. You can speak to any member of the team, call us at (281) 984-4550, or use our feedback form. We'll listen, look into it and get back to you.
